evaluation of renal histopathological changes, as a predictor of recoverability of renal function following pyeloplasty for ureteropelvic junction obstruction

چکیده
conclusions the severity of pathological changes in renal parenchyma, due to upjo, is a good predictor of renal function recoverability, after pyeloplasty. the orus, with drf > 35%, usually have normal (grade i) renal biopsy and might be expected to present better functional recoverability after pyeloplasty. results this study showed a linear relationship between the severity of histological changes and renal function recovery, after pyeloplasty. out of 24 obstructed renal units (oru), with minimal histopathological changes (grade i), 21 oru (87.5%), with > 35% drf preoperatively, showed significant improvement in renal function after 12 months of pyeloplasty (p < 0.05). on the other hand, all kidneys (n = 29) with moderate to severe obstructive changes (grade ii and iii) had minimal improvement in drf, after pyeloplasty, which was clinically insignificant (p > 0.05). renal function deterioration after pyeloplasty was not observed in any of the cases. objectives in this study, renal biopsy was carried on a series of cases of upjo, during pyeloplasty, to study the extent of histological alterations in renal parenchyma, as a result of obstruction, and its predictive value in renal function recoverability after pyeloplasty. patients and methods we retrospectively analyzed the renal biopsy obtained during pyeloplasty in 53 adult patients. histopathological changes were graded on a scale of 1 to 3, according to their severity, and compared with the differential renal function (drf) revealed on the preoperative and postoperative follow up diethylene triamine pentaacetic acid (dtpa) renal scan. a fischer’s t test was used to evaluate statistical differences between values. background pyeloplasty is a widely accepted treatment for ureteropelvic junction obstruction (upjo). however, the renal function recoverability after pyeloplasty is still a matter of debate. different parameters have been used to predict renal functional recoverability after corrective surgery, with conflicting results.

Evaluation of Renal Histopathological Changes, as a Predictor of Recoverability of Renal Function Following Pyeloplasty for Ureteropelvic Junction Obstruction
OBJECTIVES In this study, renal biopsy was carried on a series of cases o...
متن کاملCrossing renal vessel causing ureteropelvic junction obstruction.
Ureteropelvic junction UPJ obstruction is caused by the presence of an aperistaltic dysplastic segment at the UPJ. Besides this intrinsic etiology, extrinsic factors, mainly crossing vessels, maybe the causative factor. The controversy regarding the functional significance of vessels crossing at the UPJ is not a new one, though this debate has been resurrected in recent years because of improve...
متن کاملResults of Pyeloplasty in patients with Ureteropelvic Junction Obstruction in Pediatric Surgery Center in Bandar Abbas
Introduction: Ureteropelvic junction obstruction (UPJO) is associated with inadequate urine drainage from renal pelvis into ureter. UPJO results from incomplete recanalization of proximal ureter during fetal period which finally leads to hydrostatic distention of renal pelvis and calyces. Diagnosis is based on clinical symptoms, ultrasound, and diuretic isotopic renogram. Various surgical metho...

OBJECTIVE The purpose of this study was to evaluate split renal function, estimate single-kidney renal function, and identify cause of obstruction in patients with ureteropelvic junction (UPJ) obstruction by using contrast-enhanced dynamic MR renography (MRR). MATERIALS AND METHODS Seventeen patients with UPJ obstruction underwent MRR and diuresis nuclear renography. Nuclear renography assess...
متن کاملTransperitoneal laparoscopic pyeloplasty in the treatment of ureteropelvic junction obstruction
INTRODUCTION Laparoscopic pyeloplasty was first described by Schuessler. During the last decade, this technique has been developed in order to achieve the same results as open surgery, with lower rates of morbidity and complications. In this study we review our experience using laparoscopic pyeloplasty as the gold standard for the treatment of the ureteropelvic junction obstruction (UPJO). MA...

OBJECTIVES To present our initial experience with laparoscopic pyeloplasty and to evaluate the safety and short-term outcome of this technique in children. METHODS Thirteen kidney units in twelve children underwent laparoscopic dismembered pyeloplasty for the management of ureteropelvic junction obstruction (UPJO) at our institution between 2005 and 2008. Patient age at surgery was 18-177 mon...
